This contact information will be used for the student directory which will be printed by the PTA and given to all Muddy Brook families this fall. PLEASE do not include any information that you do not want to see in print. Also, please fill this out completely even if the information is the same as last year. If this is not returned we cannot include your child in the directory. Many families find this helpful when trying to contact friends throughout the year. We have added an additional space for a second contact so both parents can be included if the information is different. If you have more than one child, you may list more names at the bottom and we will make sure they are listed with their class. THANK YOU!!
